An exciting opportunity has become available to work within the Care4CE Dementia Reablement service. The position is for a permanent Grade 6 Senior Support Worker, 37 hours per week, primarily working Monday to Friday 9am – 5pm, although some early evenings may be required. The service is based in Macclesfield Town Hall , but the team are Agile Workers so can also work from home at times.

ABOUT THE ROLE

The Dementia Reablement team provide short term (up to 12 weeks) support to individuals recently diagnosed with dementia. The team provides information, advice and practical support to enable individuals referred to the service to continue to live independently for as long as possible and to continue to enjoy a good quality of life. The team provides specialist, short term interventions that are designed to equip people with early dementia, their carers and their families, with the practical skills, tools and information that can improve outcomes for the person affected.

You will have:

  • Effective teamwork and communication skills

  • Able to work on your own initiative and as part of a team
  • Able to manage a busy workload and keep to deadlines
  • Be able to visit service users and carers i across Cheshire East and support to transport to community activities
